Cancellation

If the name / certificate number of your plan belongs to:

Patriot Exchange Program / EPSNI*********
Patriot Group Exchange Program / EPSWN********
Patriot America / PATA********

The rules and method of cancellation:

1. Before the effective date of your plan, you can get a full refund.

2. After the effective date,
a) If you have claims filed with claim department, you can’t get a refund.
b) If you do not have any claim request, you will be required to pay a $50 cancellation fee, and only full month premium will be considered for refunds.

3. Send the certificate number, insured name, date of birth, reason of cancellation to cancel@imglobal.com. Email should be sent in English.

If the name / certificate number of your plan is:

Atlas America / A********

1. Before the effective date of your plan, you can get a full refund.

2. After the effective date,
a) If you have claims filed with claim department, you can’t get a refund.
b) If you do not have any claim request, you will be required to pay a $25 cancellation fee, and the unused part will be refunded.

3. Send the certificate number, insured name, date of birth, reason of cancellation and the date when you want to cancel the plan to orders@hccmis.com. Email should be sent in English.

If the name / certificate number of your plan is:

Student Secure / SS********

The rules and method of cancellation:
1. Before the effective date of your plan, you can get a full refund.

2. After the effective date,
a) If you plan has been started over 60 days, or you have claims filed with claim department, you can’t get a refund.
b) In other conditions, you will be required to pay a $25 cancellation fee, and the unused part will be refunded.

3. Send the certificate number, insured name, date of birth, reason of cancellation and the date when you want to cancel the plan to orders@hccmis.com. Email should be sent in English.
