Claim Procedures

If the name / certificate number of your plan belongs to:

DIANins Blue / Brown / Green / White
DIANins Purple/ Orange/ Pink/ Yellow

Please see the claim method as below:

  • If the hospital support the directly billing with your insurance company, and the hospital can send your medical records and bills to the claim department in time. You only need to fill out a Claim Form sending to claim department.
  • If the hospital can’t support the directly billing with your insurance company, you need to pay the full expenses first, and then send all medical records and bills in addition to a completed Claim Form to claim department.
  • Claim materials need to send to the claim department:

If the hospital hasn’t sent your medical records or bills to the claim department, it may cause make your claim processing in a pending status. Please contact your hospital as soon as possible to send the claim materials in a timely manner.

Please see the claim method as below:

  • If your insurance company can pay the bill directly to the hospital or clinic, you need to fill out a Claimant’s Statement and Authorization form sending to claim department. After receiving the Claimant’s Statement and Authorization form, claim department will verify your medical records with your hospital or clinic, and then start to your claim processing.
  • If the hospital or clinic can’t support the directly billing with your insurance company, in addition to submit Claimant’s Statement and Authorization form, you will also need to send all the relevant original receipts/bills and medical records to claim department.
