Review the waiver deadlines for the academic year, then click “Submit a Waiver” during your designated waiver period.
Answer each question based on your own insurance plan. The screenshots show where to select the plan type, Medi-Cal status, coverage access, covered services, and reason for waiving SHIP.
Continue through the remaining coverage questions, including coverage in the United States, claims office information, maximum benefit limits, pre-existing condition waiting periods, lifetime maximums, and recreational activity coverage.
Upload the required proof of insurance, including your insurance card and supporting policy or confirmation documents. Then enter your student information and policy holder information.
If the policy holder is yourself, enter your own information. Use your insurance card and confirmation documents to fill in member ID, insurance company, phone number, and other required fields.
Read the student agreement, sign the waiver form, and submit it. After submission, make sure you receive a confirmation email and keep it for your records.
