• If you paid out of pocket, you must submit your claim within 180 days of the date of service (within 90 days for FivePoints 900/500/200/100 Plans). If your claim is denied, you may submit a Claim Appeal within 180 days, and the insurer will re-review your case with any additional documentation provided.

• Claims are typically processed within 30 business days of receiving a complete submission. If you have not heard back after this period, please contact DIANins customer service — we will check the status and verify your documents.
