Before You Start

School information: Have the UCSD student ID number, student name, and date of birth ready. The relationship selection shown in the source is Self.

Insurance-card information: Have the Aetna Member ID or policy number, Aetna Group Number, insurance-company name, and member-service phone number available.

Plan information: The source uses the Autumn waiver term and selects PPO for insurance type.

Documents: Prepare the Confirmation of Coverage, insurance ID Card, and PDF plan document. Do not make up any value that is not shown in your own insurance materials.

Important: The waiver portal can change. Check every field and document requirement against the live UCSD portal before submitting.

UCSD Waiver Steps

Step 1. Open the UCSD waiver area. Go to students.ucsd.edu. In TritonLink, open Financial Tools, then choose Health Fee Waiver/RAFT as shown.

Step 2. Enter your UCSD school email. Enter the school email address and continue.

Step 3. Complete questions 1 through 4. The source selects Other for plan type, selects I do not have Medi-Cal (California Medicaid)/Does not apply to me, and selects Yes for questions 3 and 4.

Step 4. Complete questions 5 through 10. The source selections are: 5 Yes; 6 I found another plan that cost less; 7 Yes; 8 Yes; 9 No; 10 Yes.

Step 5. Complete questions 11 through 13. The source selects 11 No, 12 No, and 13 Yes.

Step 6. Medical evacuation and repatriation. For question 14, the source selects Yes.

Step 7. Upload the Policy ID image. In the Policy Holder Information section, convert the PDF ID card received by email into an image file, then upload it using the Upload option shown.

Step 8. Enter Policy Holder Information. The source shows DIANins as the insurance company. Leave Primary Care Physician (Name) blank. For the primary-care phone field, keep the default when appropriate; enter a number only when you have one. Enter the Aetna Group Number and Aetna Member ID from the insurance card. The source shows member-service phone number 800-791-4531.

Step 9. Complete address, plan, and relationship fields. The source shows: Address Redbridge, P.O. Box 144490; City Coral Gables; State Florida; Zip Code 33114; health-plan type PPO; relationship to student Self.

Step 10. Attach supporting documents. Upload the ID Card, Confirmation of Coverage, and plan document as required. Rename the files before upload by removing underscores and numbers. The screenshot also states that each file must have a unique name, the total attachment size must not exceed 30 MB, and accepted formats are PNG, JPEG, PDF, and HEIC.

Step 11. Submit and keep the confirmation. Review all fields, submit the waiver, and save the confirmation screen. The waiver remains subject to review; check the status on the portal and watch for the response after review.
