University of Chicago Health Insurance Waiver Application Tutorial

Step 1: Open the Waiver Page

Open the official University of Chicago U-SHIP enroll or waive page below:

https://wellness.uchicago.edu/student-insurance/u-ship/enroll-or-waive/

Follow the school page to enter the UnitedHealthcare StudentResources waiver portal.

Step 2: Fill In Personal Information

Complete your personal information, including name, email, student ID, date of birth, and campus location. Then click 鈥淣ext鈥?to continue.

Step 3: Answer Waiver Questions

Review the coverage requirements and answer the waiver questions according to your insurance plan. The example screenshots show the required coverage criteria and where to select 鈥淵es鈥?when your plan meets the requirement.

Step 4: Enter Insurance Information and Upload Documents

Enter your insurance type, member ID or policy number, group number, and policy holder information. If your insurance company is not listed, select 鈥淥ther鈥?and type the insurance company name.

Upload the front and back of your insurance ID card or other required proof of insurance when prompted.

Step 5: Sign, Submit, and Check Approval

Read the agreement, sign the form, and submit your waiver request. After submission, wait for the decision email from UnitedHealthcare StudentResources or the Student Insurance Office.

Once approved, keep the confirmation email for your records.
