Before starting, prepare your insurance ID card and plan information, including policy holder details, insurance company and plan name, group number, member ID, company phone number, company address, and effective date.

Step 3: Confirm the Coverage Requirements

Step 4: Enter Policy Holder Information

Fill in the policy holder name, birth date, relationship to student, country, address, city, state, and ZIP code. If you are the policy holder, choose “Self” where applicable.

Step 5: Enter Student Health Insurance Information

Use your insurance card and confirmation documents to complete the group number, member ID, plan effective date, insurance company address, ZIP code, and phone number, then continue to submit the waiver.
