USC Health Insurance Waiver Application Tutorial

Step 1: Open the USC Student Health Waiver Portal

Open the USC student health insurance waiver portal below:

https://eshc-pncw.usc.edu/

Click “Login with your USC NetID” to begin.

Step 2: Sign In With Your USC NetID

Enter your USC NetID and password, then sign in to access the waiver application.

Step 3: Fill In Waiver and Coverage Information

Choose “Student Insurance Waiver” as the waiver type and select the correct insurance term. If your insurance plan is not listed, select the option indicating that your plan is not listed or that you have other insurance, then type your plan information manually.

Use your insurance documents to fill in the policy number, group number, start date, end date, relationship to subscriber, and subscriber personal information.

Step 4: Enter Address Information and Upload Insurance Card

Complete your email and address information. Then upload clear screenshots of the front and back of your insurance ID card.

After all required fields are complete and the card images are uploaded, click “Proceed” to submit the waiver.
