UCD 2026-2027 Student Health Insurance Waiver Guide
English guide based on the source article and its waiver screenshots. Use the live UC Davis / Academic HealthPlans waiver portal and your own insurance documents as the final authority.
Start from the UC Davis SHIP waiver page: shcs.ucdavis.edu/insurance/waiver.

School information: Have your UCD login information, Student ID, school email, and your own name and contact details ready.
Insurance information: The source selects PPO for plan type and uses Member Service Phone 800-791-4531.
Documents: Prepare the front and back of your insurance card, the Confirmation Letter, and the Master Policy / SBC document. Do not invent any value that is not shown in your own insurance materials.
Step 1. Answer questions 1 through 4. In the waiver process form, follow the screenshot: Q1 Other; Q2 I don't have; Q3 Yes; Q4 Yes.
Step 2. Answer questions 5 through 8 (coverage scope and waiver reason). The source selections are: Q5 Yes; Q6 I found another plan that cost less; Q7 Yes; Q8 Yes.
Step 3. Answer questions 9 through 14 (insurance coverage details). The source selections are: Q9 No; Q10 Yes; Q11 No; Q12 No; Q13 Yes; Q14 Yes.
Step 4. Upload your insurance materials. Upload the front and back of your insurance card as shown.
Step 5. Complete the Policy Holder information (upper part). Enter your name according to the insurance documents. For the insurance company, select DIANins Global first; if it is not available, select Spectrum Life Ltd. (if the dropdown has neither, choose Other and type it). Enter the Group Number and Member ID from your insurance card, and set Member Service Phone to 800-791-4531.
Step 6. Set plan type, relationship, and attachments. Enter your current ZIP code, select PPO as the plan type, select Self for the relationship to the student, and attach the Confirmation Letter / Policy file.
Save the submission-success page or the confirmation email screenshot. A UCD waiver usually takes some processing time, so keep the confirmation and check the status on the portal.
If you are unsure how the Insurance Company, Member ID, Group Number, or uploaded files should be filled in, prepare your insurance card, insurance PDF, and waiver page screenshots, and check them item by item with a DIANins advisor.
It is recommended to submit 3 to 5 days before the deadline to allow time to provide any missing documents.
